Medication Administration

In order for your child to have medication administered at school, we will need the following required by the ODH and Board Policy:

A Medication Form (available below) must be filled out for all prescriptive medications by the doctor and signed by the doctor and a parent/guardian. Meds Can Never Be Sent in With the Student
Over the counter medications such as Tylenol and Motrin may be kept at school with a completed medication form signed by the parent/guardian. Meds Can Never Be Sent in With the Student
Medication must be brought in by a parent/guardian in the original packaging. Students are not permitted to transport medications.
Medications will not be accepted until the form is completed.

Immunization Requirements

2024-2025 Immunization Requirements – by Grade

These are required for school entry. Please give all documentation to the school office.

Kindergarten Entry:

DTaP (Diphtheria, Tetanus, Pertussis): 4-5 doses. The final dose must be given after the 4th birthday.

Polio: 3-4 doses. The final dose must be after the 4th birthday.

MMR (Measles, Mumps, Rubella): 2 doses. The first dose must be given after the 1st birthday.

Hepatitis B: 3 doses. Spacing is critical. If given too soon, a booster is needed.

Varicella (Chicken Pox): 2 doses. The first dose must be given after the 1st birthday.

7th Grade Entry:

Tdap (Tetanus, Diphtheria, Pertussis): 1 dose

MCV4 (Meningococcal): 1 dose

12th Grade Entry:

MCV4 (Meningococcal): 2 doses - if the first dose was given on or after the 16th birthday, a second dose is not required.

Immunizations may be administered through your family physician or:

Area pharmacies such as Meijer, Rite Aid, Kroger, Walmart, etc.
Ottawa County Health Department - has a monthly clinic in Genoa. Call 419-734-6800 to schedule an appointment.

Shots 4 Tots n Teens (Lucas Co. Health Dept.) - has a clinic at St. Charles Hospital each month. Call 419-213-4121 or visit lucascountyhealth.com click “Health Services” and then “Child Immunizations” for schedule of clinic times and locations.

These facilities require a parent to be present and a past shot record.
