Immunization Requirements Kindergarten & 7th grades

The immunization requirements have changed for the 2010-2011 school year. Please refer to the list below.
2010-2011 School Year
Kindergarten Entry
DPT(or any combination)-5 doses if the 4th was given prior to the 4th birthday
Polio (any combination)-4 doses the final dose must be given after the 4th birthday or a booster will be needed
MMR -2 doses The first dose must be given after the first birthday
Hepatitis B-3 doses timing is important here. If the timing is off a booster will be required
Varicella (Chicken Pox)-2 doses prior to kdg entry or a signed note of having the Chicken Pox
Also required for kindergarten entry are:
Birth certificate Social Security card Immunization record Doctor's physical for kindergarten dated in current school year Dental exam dated in current school year Custody papers (if appropriate)
7th Grade Entry
DPT (or any combination)-1 dose a booster is required prior to entry into 7th grade.
